Job Summary

The Dietitian Case Manager is responsible for assessing the individual nutritional needs of patients, establishing and monitoring nutrition care plans, facilitating nutrition topics and providing medical nutrition therapy and documenting all assessments.

Specific Responsibilities

  • Provide dietitian counseling over the phone or virtual to patients.
  • The management of the assigned projects and/or cases; participate in the evaluation of current program and implementation of new programs.
  • Liaises with patients/ caregivers or healthcare professionals via telephone or virtually.
  • Planning, organizing, directing and evaluating patients’ nutritional care, including assessments and reassessments in collaboration with our interdisciplinary team, the patient, the patients’ family members, caregivers and Physician
  • Provides medical nutrition therapy virtually or via telephone within topics identified by interdisciplinary team.
  • Administer and adhere to case-management processes, including clinical monitoring and data collection
  • On-going patient follow-up, adherence, therapy monitoring, adverse event reporting
  • Support and collaborate with the PSP staff in the development & implementation of disease management programs and medical nutrition therapy
  • The Dietitian Case Manager will also be assigned other duties and tasks as required from time to time.

Experience and Education

  • Bachelor of Science degree in Foods and Nutrition.
  • Current membership with the Order of Dietitians in current province.
  • Familiar with trauma-informed care and weight inclusive approaches, including mindfulness.
  • Registered Dietitian within a Hospital, Clinical or Pharmaceutical setting experience
  • Knowledge of Metabolic and Genetic Disorders.
  • Pediatric experience an asset
  • PSP Case Coordinator experience an asset
  • Fluently bilingual in speaking and writing of the French and English language is preferred
  • Travel may be required, depending on program needs.

Minimum Skills, Knowledge, and Abilities

  • Demonstrated knowledge, skills, and abilities, dietetic assessment, treatment, and counseling a wide age range of a patient population
  • Communicates with clarity, verbally and in one on one or group situations, or over the telephone.
  • Communicates well in writing by composing clear documents; facility with editing and/or proof-reading is required.
  • Excellent problem-solving and decision-making skills,
  • Adaptable to set and prioritize work independently. Able to work with diverse personalities and styles.
  • Professional manner, tact, diplomacy, and discretion in dealing with associates and colleagues including senior management.
  • Effective organizational skills: attention to detail and ability to prioritize and manage time effectively with multiple projects
  • Knowledge of Microsoft Word, Excel, PowerPoint and Outlook.
  • CRM or electronic medical record experience an asset.

The pay range shown below is aligned with McKesson's pay philosophy, and pay will always be compliant with any applicable regulations. Our Base Pay Range for this position

$53,800 - $89,600
