Job Summary


Ensures the SPS pharmacy team provides optimal service and pharmaceutical care to its patients and physician customers.  Works with pharmacists to ensure therapeutic area expertise, promote development and develop programs that promote practice excellence and improve the patient and physician experience.

This position also ensures the safe and effective use of medications by serving as an escalation point for medication order verification while assessing for optimal drug product selection, dosage regimen design, effective administration and drug therapy monitoring services, and works with the treating physician to find alternatives when needed, to ensure the best care possible is being given.

Specific Responsibilities
 Works with manufacturer partners to develop clinical programs for pharmacists for all specialty products.

  • Maintains subject area expertise in key areas including oncology, hematology and immunology.

  • Provides direct-patient care (minimum 25%) and serves as an escalation point for complex patient care issues.

  • Prioritize level of care based on critical needs.

  • Prevent, detect, and resolve medication-related problems and to make appropriate evidence-based, patient-centered medication therapy recommendations.

  • Determine and resolve any medication therapy problems including drugs without indications, medical conditions for which no therapy is prescribed, therapy inappropriate for medical condition, incomplete immunization record, inappropriate dose, dosage form, duration, schedule, route of administration, method of administration, therapeutic duplication, medication allergies, and clinically significant drug-drug, drug-disease, drug-nutrient, or drug-laboratory test interactions.

  • Effectively verify patient medication orders for appropriateness and accuracy.

  • Appropriately document clinical interventions.

  • Provide concise, applicable, comprehensive, and timely responses to requests for drug information from patients and health care providers.

  • Report medication errors and adverse drug reactions.

  • Enhances the quality and delivery of care.

  • Actively participates in the development and implementation of performance/quality improvement and patient safety activities for Pharmacy.

  • Participate with Clinical Coordinator, clinical team and the Pharmacy department to evaluate and measure the impact of new approaches that have been implemented.

  • Participates in clinical guideline development, critical pathways, disease management, and drug use programs.

  • Represent Pharmacy/service areas on institutional/ practice area committees and quality support teams.

  • Promotes continuing education in the pharmacy and develops pharmacy certification programs.

  • Participates in mentorship and professional development of Clinical and Staff Pharmacists, Technicians, Pharmacy residents and Pharmacy students.

  • Participate in the development of new programs, services, and practices, educational activities of the Pharmacy department and medical team.

  • Consistently attend Pharmacy student/residency seminars and presentations.

  • Provides at least one (1) Pharmacy staff development initiative per year.

  • Maintains quality standards for pharmacy practice and patient experience.

  • Able to provide in-services/lectures to health care professionals outside the pharmacy department.

  • Seeks out research opportunities.  

General Responsibilities


• Filling, checking and dispensing medications accurately.
• Provide patient counseling to new patients.
• Perform patient follow-up calls.
• Provide medical information as required to patients and other health care professionals.
• Ensure prescription pricing is accurate and payment methods have been established.
• Ensure medications are delivered to the appropriate site as per patient requests.
• Provide general customer service for patients and health care professionals.
• Support the Mission, Vision and Values of the Department of Pharmacy.
• Engages in professional self-development and completes required continuing education credits and licensure requirements.
• Other duties as assigned.

Competencies

• Must be licensed and in good standing with the Ontario College of Pharmacists.
• Minimum 5 years experience as a Clinical Pharmacist; preferably with experience in oncology and hematology.
• Strong communication (both verbal and written) and presentation skills are required.
• Bilingualism (English/French) an asset.
• Independent and self-directed.

Our Base Pay Range for this position

$86,600 - $144,300
